No one goes to work to cause harm, particularly in healthcare and so uncovering or revealing unintended harm is the first step towards understanding and remedying the causes that we have failed to appreciate.
Through structured case note review organisations can ‘see in detail’ problems that have, until now, been difficult to observe. Clinicians and managers can then focus their improvement efforts in areas where they can make significant progress to improve patient safety and reduce unintended harm.
At the Quality Improvement Clinic we have first hand experience of developing and testing Trigger Tools and other interventions to increase safety and reliability in healthcare. We are also skilled in implementation, measurement and spread of effective solutions.
